What should a performance appraisal form include?

How long should the form be?

Use the minimum prompts required for the decision. Different review types and roles may need different questions; avoid repeating data already available in connected records.

Should ratings be required?

Only when the organization has a defined scale, purpose, and consequence. Require explanation for consequential ratings and retain the evidence and rationale.
